Waupun Employee Wellness Clinic Logo

In partnership, the Waupun Area School District and the City of Waupun, are pleased to provide an employee wellness clinic.  The clinic is designed to improve access to care and lower out-of-pocket costs for eligible employees of the two organizations.

Employees of the Waupun Area School District and the City of Waupun may access the clinic as follows: 

  1. Benefit-eligible employees and their dependents (children age 2+ for acute care, and children age 5+ for well-child exams), whether enrolled in their employer’s health plan or not, may use the clinic for routine wellness and preventative care and treatment of acute illnesses.

  2. Regular part-time, non-benefit eligible employees, but not their dependents, may use the clinic for treatment of short-term illnesses.

  3. In addition, occupational health services will be provided for all employees through the clinic.

The clinic will be staffed by a group of dedicated SSM healthcare professionals and provide same-day access to care whenever possible.  Clinic staff will have access to your existing medical records and will work with your primary care provider to offer personalized care.  They will also help you navigate specialty healthcare access when necessary.  

In-person appointments at the Waupun Employee Wellness Clinic incur a low fee of $20, if non-preventative or non-work related, and $5 for lab work. Virtual visits with the Waupun Employee Wellness Clinic will be at no cost.

The clinic is located within Waupun Memorial Hospital at 608 W Brown Street on the first floor.

Clinic Hours of Operation: Monday – 7:00 am – 1:00 pm

Tuesday – 11:30 am – 5:30 pm

Wednesday – Closed

Thursday – 11:30 am – 5:30 pm

Friday – 7:00 am – 1:00 pm
