Dodge County Health and Human Services: Community Services Program (CCS)
The Comprehensive Community Services (CCS) Program is a community-based psychosocial rehabilitation program designed to provide a wide variety of case management services, as well as assistance in accessing a variety of other community services and supports. The program is available to individuals with a diagnosis of a mental illness or a substance abuse disorder who also have Medical Assistance benefits. Most importantly, the focus is on empowering consumers to take an active role in their own treatment and recovery process. Services are consumer-centered and consumer-directed, meaning that the consumer has choice in the selection of those on their Recovery Team, what services they receive, and who provides these services.

Dodge County Mental Health and Substance Abuse
The Outpatient Clinic provides mental health and alcohol and drug abuse services to
individuals of all ages.
The following services are included:
- Initial assessment to include a social history and diagnostic interview to determine services needs. Individual, couple, and family therapy.
- Psychiatric evaluation and medication management.
- Case management services.
- Assessment for inpatient mental health services such as psychiatric hospitalization with a need for stabilization of symptoms, adjustment of medications, and return to community safety is indicated.
- Alcohol and drug abuse treatment.
- Counseling for addiction or abuse.
- Assessment for contractual services such as medical detoxification and referral to halfway house/residential treatment.

LeAnn Vice-Reshel, MA, LPC is available at Waupun Hospital on Fridays.
LeAnn provides compassionate care to children and adolscents. She is trained in integrative psychotherapy, tailoring the therapy to fit the patients needs. She utilizes behavioral, social skill building, parent training and trauma-focused treatment approaches. She works with behavior problems, anxiety, mood disorders, attention and concentration difficulties, crisis intervention, developmental disability, learning disability, social skill difficulties, as well as self-esteem concerns.
Kristy Sook is available at Waupun Hosptial on Thursdays and Fridays.
Individual, couples and family therapy with adolescents(13+) through geriatrics. Relationship/Interpersonal issues, depression, bipolar, anxiety, stress management, anger management, domestic violence, co-occurring disorders self-esteem issues, grief and loss.
Agnesian Fond du Lac Hospital have an Adolescent Child Psych. The Fond du Lac Campus also has Alcohol and Other Drug (AODA Services) available. Contact Intake number above for more information.
Fond du Lac Coordinated Family Services Program (CFSP)
The Coordinated Family Services Program (CFSP) strives to provide intensive services to families whose children are experiencing persistent mental health and behavioral challenges. The goal is to assist families in developing a plan of care that will help families maintain their children safely in their homes, schools and communities through a comprehensive, coordinated interagency system of care.

The Transitional Living Program - Fond du Lac County
The program is dedicated to providing comprehensive service to young adults between the ages 15-21. These services can include case management, referrals, assessments, and mentoring to enhance those youths' skills in areas such as money management, job seeking and maintenance, interviewing, cultural awareness, health and hygiene and accessing needed community resources.

Fond du Lac Comprehensive Community Services Program (CCS)
The Comprehensive Community Services Program is new to the Fond du Lac County Department of Community Programs. CCS is the latest administrative rule created by the State of Wisconsin Department of Health and Family Services for persons with severe and persistent mental illness and/or substance abuse addiction.
CCS operates on the mental health “Recovery Model,” is based on collaboration, on “doing more of what works and less of what doesn’t”, and is consumer-centered as well as strength- and outcome-based.
